Early 50's F, nonsmoker, exercise 4x/week (rowing, elliptical) with recent post pandemic weight loss, now with skin sagging and extra thigh tissue. Weight loss over 1yr. 5'3", 128 lbs. I had an extended abdominoplasty 10 yrs ago, holding up well, scars on upper buttocks. Could a buttock lift be incorporated in the same area of the scars without excessive concern for wound dehiscence? I don't want infragluteal scars. Can a thigh lift be incorporated as well?

Answer: Buttock lift Thank you for posting your question here!My answer will be based on the limited information and pictures provided, for a detailed and personalized plan please consult with your plastic surgeon. From your pictures it appears that you have some volume loss in your buttocks in addition to the drooping. using your extended abdominoplasty to perform a traditional buttock lift is likely feasible but doing so alone will likely give you a more flat buttock look , and it might not address all your concerns at the infra-gluteal fold, additionally given your previous extended abdominoplasty , you might not have the tissue to perform auto-augmentation using flaps . I would likely recommend restoring volume , either through an implant or fat grafting , and if you definitely don't want the infra-gluteal fold scars then attempting a traditional buttock lift if it is still needed after, when it comes to your thigh lift , your lateral thigh will likely improve with the buttock lift procedure , as for the medial thigh , it seems that most of your laxity is in your upper medial aspect which will likely improve with a groin incision based medial thigh lift. Answer: Butt lift….? Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made. Buttock “sagging” is usually a combination of loss of volume or deflation (after weight loss, liposuction, or post-BBL) and/or the effect of aging and gravity. So, I personally first treat patients like you by restoring volume to the buttock with a buttock implant. This will create a more desirable smooth and rounded perky buttock profile. If not adding volume with an implant, the buttock will most certainly always appear much more flat with just a buttock lift from above. A lower buttock tuck, if still needed/desired, may then performed at six months following implants. A lower buttock tuck, coincidentally, also doubles as a thigh lift. In this sequence, the implants have fully settled into final position so that any remaining loose skin can be most accurately identified and removed with the lower buttock tuck. Please note, because an extremely few number of board certified plastic surgeons specialize in buttock implants, you may have to travel away from your local area to seek consultation with this particular type of surgeon. Getting it done right the first time around is not only the best chance for success, but also will save you from a lot of headaches and money in the future. Glad to help.
