Answer: Not a good idea There are quite a few red flags in your question set that would concern me if I was your surgeon. Subglandular silicone implants: have the highest rate of capsular contracture of all possible options450 cc subglandular implants: are almost certainly too big for even tight skin envelopes to handle effectively over time, let alone in one that is already showing signs of ptosis; you have mentioned that you are afraid of a ball in sack effect, but are choosing the most likely scenario that will cause such an effect.I would highly suggest either considering a smaller implant and also placing your implants under the muscle for a more predictable and long lasting result.

Answer: 450 mL implants Thank you for the pictures. It is hard to tell how she will respond to breast implants. Based on the picture I think that you should be able to handle that volume. Best way to get a true assessment would be to have an in-person consultation with a board-certified plastic surgeon.
