Surgical hairline lowering - do people with fine, light hair have an increased risk of temporary and permanent hair loss after surgery? Who is at greatest risk for losing hair after surgical hairline advancement?

Shock hair loss is a risk with any hairline lowering surgery.  It does not matter if you have fine hair or thick hair.  Having fine hair probably would be more noticeable if you do have shock hair loss.

Your risk of hair loss with fine hair should be the same as anyone else, but fine light hair does not disguise a scar as well as coarser hair.  Overall, patients with fair skin tend to have better scars, and they are less prone to keloids or dark scar discoloration.  Patients with fine hair often have fair skin, so their hair is worse at hiding scars, but they are less likely to have very visible scars so it is often not as issue.  If you have fine hair, though, and know from experience that you form prominent or dark scars, then you should discuss that with your surgeon.  I advise that type of patient it's possible they would need a few hair grafts in the future (normally I wait about 6 months after surgery to confirm that the scar is still visible).  They will typically need a hundred grafts as opposed to the thousands that would be needed to lower your hairline with grafts alone.  -Dr. Rolfes

People with fine hair do not have and increased risk of temporary or permanent hair loss after surgical hairline lowering. This problem is very rare and is dependent on the technique used including the placement of the incisions.

The number of sessions varies by the thickness of the hair. For women with other than fine hair, they might do this in a single session. With finer hair, it often takes two sessions as my wife had done because of very fine hair. The time to see the impact of a hairline lowering hair transplant procedure is 7-8 months and if a second session is required, it will take another 7-8 months to wait for full growth.
