Hanging columellar or malpositioned alar and bulbous tip? How to fix?

I had a revision rhinoplasty with Rib grafts in alars,speader grafts and strut columellar to fix my support and form and to fix the protruding colummella. After surgery my tip is more boulbous and shift,rib grafts are very large and infratip área is curved shift and prominent and the septum protruding out with the smile. the columella and infratip are more prominent in frontal view with smile and looks down .why and which has been the fail?how to fix all this?sorry for my english

Hi Charmaine, It is important to have your surgeon evaluate you and make sure you do not have an infection. Infection rates are a bit higher with revision and if synthetic grafts were used. I rarely use columellar struts because of the problems patients have with these grafts. There are other approaches to fix columellar position and tip projection. I recommend you see an experienced revision rhinoplasty specialist.
