I had a small blemish injected with kenalog-40 and within about 4-5 days a depression in the skin appeared. It has now been about 3 weeks with no worsening or improvement... I care very much about my appearance and having this depression in a highly visible spot (my forehead) is very distressing. Does it have hope of filling in on its own? Or should I get filler now?

There is no reason for Elavil or amitryptiline to react with Botox or the dermal fillers. However, note that aminoglycoside antibiotics (gentamicin, neomycin, tobramycin, amkacin, etc.) can cause a problem here. These medications can cause the Botox to be blocked and therefore not...
It hasn't been documented that hyaluronic acid fillers can move on impact, however you may want to consider a biostimulator, such as Sculptra instead. Sculptra helps to generate your own collagen after a series of 3-4 treatments. Be careful and wear head gear too!
Laugh lines can be treated using hylauronic acid based fillers like Restylane Refyne and Restylane Defyne. However, I would not treat your dimples themselves.
