Buccal fat removal 2 years after, still not happy. About to get it for the 2nd time, any suggestions? (Photos)

Tried all sorts of things, diff surgeon did a fat burning laser on cheeks+neck lipo, no difference. Fillers injected on upper jaw line, lips chin, those look OK but don’t fix my problem. My cheeks are CHUBBY when I smile. Orig surgeon says I dont need a 2nd buccal fat pad removal but he would do it if I want. My cheeks feel chubby, thick, is it regular fat? Skin? Is it possible I still have buccal fat there? What other procedures could help me? dimpleplasty? Cheeks lipo exist? I'm 136lb 5 10''
