I'd like to get a chin implant but am unsure if it will look natural or not with my mental crease. Can a chin implant bring that part forward as well? Could I get a implant and then after get some filler put in there? I had rhinoplasty and neck liposuction 7 months ago, but now want to improve my chin as well.

Thank you for your pictures.  Your neck and nose look very good.  I would not recommend a chin implant as this might accentuate your mental crease.  A genioplasty would be a better choice because it brings the soft tissue with more reliably in my opinion.

A chin implant is not designed to bring the mental crease forward.  The Mental crease is directly over the tooth roots and the implant does not need to ride on the root of the teeth. Chin implants are manufactured and number  of sizes and shapes, are composed of Silastic, and are usually inserted under local anesthesia as an outpatient procedure. the size and the shape of the implant is determined at the time of examination and consultation.  A chin implant can always be removed if the patient so desires.

The labiomental crease is a fixed anatomic point that will not change regardless of whether a chin implant is placed or a sliding genioplasty is done. Fortunately your chin augmentation requires as much vertical lengthening as it does horizontal projection. This type of chin movement will minimize any negatve effect on the labiomental crease. Injection fat grafting can also be done on the crease to help prevent it from deepening in any type of chin augmentation.

Although the chin implant does not effect the mental crease, you would benefit from a chin implant.  A chin implant will project the chin forward to better balance the lower half of your face.  Your mental crease  is appropriate.  It should be 5mm deep...and yours is approximately 5mm.  This procedure can be performed under local anesthesia.

Chin implants will usually efface the horizonal crease obviating the need for a filler. If a filler is  needed, one can be chosen that suits your needs.

The most ideal approach is a sliding genioplasty which allows the placement of a small amount of implant material (gortex,hydroxylapatite, etc) on the 'ledge' of bone created; which will easily soften the fold. Chin implants are less ideal in that case. As well, the sliding genioplasty will 'stretch out' the muscle under the chin area and 'flatten' it nicely; whereas a chin implant is just an 'add on' and cannot affect the muscles in the same way.

There are implants designed to provide for better transition from the chin prominence to the lower lip. In extreme cases injectable filler might be tried later as could the insertion of a soft silastic subcutaneous implant. See how your chin-lip profile looks on computer imaging on smiling, open mouth  and resting views. If extreme projection is not sought, the chin-lip transition with the proper implant may be all you need.
