My back isn't extremely fatty and I am going in soon for a breast reduction and I have pinched the skin under my breast how a lift would be done and it basically gets rid of the roll. Is it safe to assume that after surgery it will be tighter?

Breast Reduction does just that, it removes breast tissue, skin, elevates the nipple, and "lifts" the breast. It is not intended to help the back or sides. With that said I usually liposuction the area lateral to the breast on the side of the chest with every reduction. Many women with large breast have a roll of fat there and it looks funny if not liposuctioned at the same time. It adds maybe 5 minutes to the case and is done with syringe liposuction cannulas. If there is loose skin it may not help that, and additional surgery, not covered by insurance for breast reduction may be needed.

Answer: Bra rolls and breast reduction
Bra rolls are a combination of fat and loose skin. A breast reduction wil not help this. A direct excision or liposuction can.

Answer: "Bra" rolls on the back and breast reduction
A breast reduction removes breast tissue and skin to reposition and reshape the breast but does not actually "tighten" the skin and has no lasting effects outside the surgical area.
Liposuction, including ultrasonic liposuction, reduces fat cells and does not tighten the skin.
Rolls in the back or along the bra line beyond the breast may be helped somewhat by liposuction if there is excess fat in that area. If it is a skin or connective tissue laxity as in skin and connective tissue sagging from the underarms, the only reliable treatment is excision and trading for a scar.
