For reference I'm 28 (f), no children, 5'9", and currently a 36D/38C. My goal is more upper pole fullness and projection. I am not a afraid of the "fake" look. I have sent images to a doctor for a virtual screening and they are recommending a lift. I don't believe I require a lift based on reviewing before and afters of other women. What would you recommend (lift or no lift, over/under, silicone/gummy bear, and what CC's)? Thanks in advance!
Answer: Considering options Although an in person examination is always best, from your photos it appears you would have the best results from a breast lift with implants. This would raise the position of the nipple as well as give you the volume increase you are wanting. If you create upper pole fullness with the nipple low you may end up with an overall shape that you won't like.

Answer: Implant surgery alone seems not enough It seems that implant surgery alone will not result in a good shape. In your case constriction and ptosis are more severe than in the cases of others attached, so peri-areolar mastopexy seems to be necessary. A silicone implant will be used, but the size can be predicted to some extent through examination, and since mastopexy is used together, it is desirable to make the final implant decision during surgery because the shape and size should be simulated by using a sizer, a temporary implant during surgery.

Answer: Breast Augmentation - upper pole fullness Unfortunately I agree... do a lift. In fact I would consider doing your case in two stages: stage 1 breast lift then stage 2 breast augmentation. I would separate the two stages by 3-6 months. Best Wishes!!

Answer: Breasts You need a lift. Your breasts are sagging. If you just add implant, you will still have sagging but larger breasts. Get the lift with implants placed behind the muscle for you best results. Liposuction of the axillary fat would also work well for you.

Answer: Breast lift Thanks for your question! To get the best surgical outcome, I agree with doing a breast lift. If you just do an implant, your breasts will be larger but will still appear heavy and droopy. I might advise seeking a second opinion in person. Good luck!

Answer: Bellesoma Method If you are already a 36D/38C then implants will not solve your problem. Your breasts are too low on the chest wall and pointing down and out. The technique I recommend is a lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers or small implants can be placed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.
