To make a quality assessment regarding the outcome of any plastic surgery procedure we generally need to see a complete set of proper before and after pictures.If you’ve had more than one procedure than we should ideally see before and after Pictures of each of your previous procedures.If you don’t have before, and after pictures, then contact your surgeon and ask them to forward the pictures they took.The majority of plastic surgeons in the United States do not offer gluteal implants.They seem to be happy listing every other Plastic surgery procedure on their websites whether they have experience or not.Gluteal implants is a procedure that has some of the highest risk of complications, undesirable side effects need for revision surgery, and ultimate failure.There are few plastic surgeons who perform the procedure and much fewer who have The skill and experience to deliver reasonably good outcomes.In my opinion, the majority of patients who get gluteal implants will eventually either lose their implants, have the implants removed or eventually be regretful of having had the procedure in the first place.In other words, the chance of having long-term high satisfaction outcomes is even in the hands of the best providers pretty low.Grafting fat is really not the solution to a poorly placed implant or an implant that is incorrectly sized for the anatomic space.Each time someone has a revision procedure the situation becomes more more complex and the chance of having a high Quality outcome goes down.Repositioning the implant is much more difficult than placing it correctly during the primary procedure.My best recommendation for patients is to stay away from gluteal implants if possible.Even a modest improvement from fat transfer still seems to be a better option.For those who do for gluteal implants provider selection becomes Paramount.I think it’s going to be challenging to make significant improvements on your outcome.To get quality, second opinion consultation, I suggest you schedule those as in person consultations with providers who have ample experience with gluteal implants.For second opinion, consultations, come prepared bringing with you a complete set of proper before and after pictures and copies of your previous operative reports.These should all be available from your current provider if you request them.They are all part of your medical records which you have a right to get a copy of. I don’t think there’s an easy solution to your problem and that’s why providers are giving you vague answers.Sometimes it’s better to “cut bait” and recognize that further surgery simply isn’t in your best interest and there are inherent limitations to what can be accomplished.As is the case for so many people provider selection is the most important variable so I suggest focusing your efforts on that variable first.To find the right provider is not at all easy or straightforward.It generally means having in person consultations with plastic surgeons who have proven track record with this procedure.It can be time consuming and potentially involve cost. I generally always recommend people avoid virtual consultations and always have multiple in person consultations before considering selecting a provider or scheduling surgery.The more complex the situation the more this is justified.Hopefully, you’ll be able to find someone who can improve your outcome.Best,Mats Hagstrom MD
