In order to make an assessment on the outcome of a plastic surgery procedure we need to see proper before and after pictures. Gynecomastia is often slightly asymmetrical with one side being worse than the other. The human body is generally quite asymmetric.
There are three tissue variables that can contribute to lack of ideal results. Those are skin laxity, glandular breast tissue or subcutaneous fat.
Pectoralis muscle does not contribute to gynecomastia.
To make an accurate assessment of the contour of underlying soft tissues try putting the pectoralis muscle and your skin under tension by taking one arm at a time and stretching it behind your head.
While doing this look in the mirror or take a picture using the timer on your camera or have someone else take the picture.
With both pectoralis muscle and skin under tension you can clearly see if there is persistent glandular tissue or subcutaneous fat.
You can also watch the contour reform as you slowly put your arm down to see how much the contour changes without the skin being under tension.
Depending on if the problem is skin, fat or breast tissue determines if revision surgery is indicated or not.
