Will lipo help or will I need a back lift? (Photos)

I had an FDL panniculectomy in 2023 and I’m happy with the results in my lower abdomen (as can be expected from this procedure) but not so happy with the residual fat and skin laxity in the upper abdomen (again- as expected.) When I first made contact with my new surgeon, I was hoping to revise my fdl panniculectomy to a full tummy tuck. During our ****zoom**** consultation (i am out of state,) the doctor expressed apprehension to give me a tummy tuck because we aren’t working with a “virgin” stomach (again- completely understand) and he recommended doing lipo 360 WITH Jplasma and a bbl to give me a more feminine shape. However - he did mention that maybe I would have to have a bra line back lift in the future after this lipo procedure because it would potentially cause *more* loose skin. He said consulting over zoom made it more difficult to promise results and he would have an easier time distinguishing what he thinks will happen when i meet him at my pre-op appointment. I am pretty committed to having this procedure and I am excited to amplify my natural curves with a little something extra. I wanted to come on here and get some opinions….should i *start* researching doctors for the back lift? is the lipo going to do **anything?**

Based on your history and photos, your surgeon’s approach is reasonable given the limitations of a prior FDL panniculectomy, where achieving full upper abdominal tightening with a standard tummy tuck can be more challenging. Liposuction combined with J-Plasma can improve contour and provide some degree of skin tightening, but it will not replicate the results of a formal excisional procedure, especially in cases of residual skin laxity. It may enhance your shape and definition, particularly when combined with fat transfer, but mild to moderate looseness can persist or even become more noticeable after fat reduction. For this reason, the possibility of a secondary procedure such as a bra-line back lift or upper body lift is sometimes anticipated. Overall, your plan can deliver improvement, but expectations should remain realistic, and a staged approach is often the safest and most effective strategy in revision cases.

Skin retraction after liposuction and  skin tightening (renuvion) is very unpredictable, based on your skin laxity. A combination of circumferential torso liposuction and direct excision of skin folds,  possibly skin tightening with Renuvion, would be my first option. It is quite possible that you may need additional surgery like revision abdominoplasty if this does not turn to be sufficient? I hope this helps. Good luck.

Thank you for your questions and pictures.  From what I can see, and again being difficult without an in person exam, I do believe that you would be a good candidate for and in need of a back lift, or bra line lift for the skin excess.  As for the upper abd skin excess, I would be comfortable with additional skin excision using probably only the vertical scar.  Be sure to see a Board-Certified PS preferably in your area for you consultation.
