My boobs lean to the side when I lay down and I hate it. What procedures can fix this?

A breast lift will make your breasts perky and should correct lateral displacement of the breasts.  Some amount of reduction may also be needed.

best advice you can receive is from an in-person consultation.  Based on
your description, you may benefit from a breast lift.  This decision should
be based on a detailed discussion with equal input from both you
and your surgeon.   Make sure you specifically look
at before and after pictures of real patients who have had this surgery
performed by your surgeon and evaluate their results.   The most
important aspect is to find a surgeon you are comfortable with. I recommend
that you seek consultation with a qualified board-certified plastic surgeon who
can evaluate you in person.

Hello and thank your sharing your question. From your drescription it seem w=you would need a Breast Lift / Reduction. With or without implants is hard to say, as is the type of lift, without at least seeing some photos. Please follow-up this question with front and side view photos.
