Hi folks. I am hoping you can give me some advice on how to achieve a much more natural look and slope to the breast from what I have. I am getting revision surgery, and plan to go for motiva ergonomix implant, possibly a bit smaller and under the muscle? Currently I have round 335cc above the muscle. My surgeon and I just don't seem to share the same vision so I feel I have to go to him with exactly what I want. I have included pics of my current breasts and what I wish to achieve. Thanks

Exactly how/where the breast implant "pocket" is dissected and which breast implant size/profile is selected may play a big role when it comes to final aesthetic outcome achieved. My best suggestion: select your plastic surgeon carefully. Make sure that he/she is a board-certified plastic surgeon who can demonstrate significant experience achieving the types of outcomes you would be pleased with. Then, communicate your goals carefully as well. The use of goal photographs and computer image may be very helpful during the communication process. In my practice, I would ask that you utilize nude goal photographs. After doing so, your plastic surgeon will be able to tell you whether or not the desired outcome is realistic, given your specific anatomic starting point. Best wishes.

In order to obtain a more natural shape and size of the breast, I would agree with having new, smaller implants placed behind the muscle. You may also have some scar tissue around your current implants which would need to be removed at the time of surgery. I would ask your surgeon if he is able to provide something similar to the results you hope to achieve; very few of us can create breasts as perfect as those of Monica Bellucci! Some surgeons have extensive experience with primary breast augmentation, whereas others have more experience with secondary (revision) breast surgery, so you may need to visit another plastic surgeon in your area for another opinion. I understand and completely agree. First off we have to determine why your current result looks as it does. It could be that the implants were simply too large for your original breast size. Without pre-op photos I cannot be sure, but I think probably not. The other explanation is that you have capsular contractures. This is especially likely given that your implants are above the muscle, but without an exam I really cannot be sure. If the issue is that the implants are simply too large, then the solution is smaller implants. If the problem is capsular contracture, then removal of the capsule (capsulectomy) and switch to a textured implant or switch to a sub-muscular plane are reasonable solutions.As for you and your surgeon having different visions: you can attempt to correct that by being more precise during your consultations. But I would be quick to choose another surgeon if you are not absolutely confident that you and your original surgeon are on the same page.Good luck.
