Male in late 30s; had lipo on chest and stomach with good results, but have gained fat in in my back and love handles.

Question

Now, interested in having this fat removed, but concerned that following the procedure the fat will return in another area (visceral and/or face, arms..etc.). I'm curious if fat grafting to another area - similar to how woman may have thigh lipo and use the fat to augment their breasts..etc.. I am curious if that is done in men, as it is in woman, and if so, where are some areas where the removed fat may be grafted/reinserted?

Thank you for your question. You sound like a good candidate for repeat liposuction. And yes, the removed fat can often be used in men, just like in women, to augment areas in need. However, the differences in men and women in terms of fat grafting are very different and need to be understood and appreciated. For example, in women, grafted fat into the soft tissues (breasts, buttocks, etc) are soft and pliable and for women that is great. For men on the other hand, you do not want something soft that feels unnatural on a male body. So often the fat needs to be grafted into muscular areas in specific ways to produce nice results. Make sure you consult with a local board certified plastic surgeon who appreciates this nuance and has good experience in this procedure. Best of luck!

Most men in my practice request abdomen, sides, back, and chest liposuction.  Although an examination is always helpful, liposuction of the love handles usually produces a very nice result.  The result has much more to do with the surgeon than the type of liposuction performed.  The fat that is harvested can be grafted to your biceps, butt, calves, face, or pecs.I would search for the board certified plastic surgeon who you feel is most likely to give you the best results and who performs hundreds of these procedures each year. I would look at reviews and before and after pictures on that surgeon's website.

When one has liposuction, its advisable to have all the areas of lipodystrophy treated. Sometimes what appears to be new areas of fat, may simply be areas not initially treated since they were not as prominent or noticed. At other times, since we lost our usual barometers for observing weight gain such as the stomach, pounds will creep up on us and get more noticed  in areas that were never treated, such as the flanks and back in your case. In the treated areas, such as the chest and abdomen, where  fat also accumulates as pounds are added, it may be slower to be noticed. The treated areas will take time to reach their preoperative objectionable  appearance.  It is very critical to rely on a scale post liposuction and maintain one's weight steady, and not rely on one's appearance alone.
The fat removed from the abdomen and flanks and be used to augment areas which are cosmetically more appealing when volumized for aesthetic balance.  Fat grafting is provided for men to achieve high cheek bones, prominent chins and jawlines, enhanced biceps and pectoralis (chest) muscles, enhanced penile girth, more pronounced gluts (buttock)  and enhanced gastrocnemius (calf) volume.

Hello and thank you for your excellent question. Based on your description, you are certainly a candidate for additional liposuction. With regards to fat grafting in men, it is possible to have fat grafted to many different areas. This may include your pectoral muscles, calves, biceps, facial fat grafting, or even penile fat grafting. Of course, you will need an in-person consultation to evaluate you for this procedure. Preoperative visualization and surgical simulation with the Crisalix system may help with your decision. Any treatment plan should be based on a detailed discussion with equal input from both you and your surgeon.    Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results.   The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation in person with a qualified board-certified plastic surgeon. Good luck!

After a liposuction you simply cannot gain much weight.   Weight will always go somewhere.  Weight can collect around the intestines, on arms or back,  or any area not treated.   Fat transfer in males is less common than in females but fat can be transferred to the face or to any deficient area.    My Best,  Dr Commons
