I am no longer a fan of fat injections, having stopped using this technique 20 years ago. Fat transfer requires surgical interventions, one to extract the fat from the buttocks, thighs or belly, and the other, using large bore cannulas, to reinject the fat into the face. Unfortunately, despite the theory that fat taken from one part of the body would stay around for a long time when injected into another part of the body, the theory didn't seem to consistently pan out. To boot, the procedure was expensive and had a well-recognized downtime of significant swelling and bruising. Today we are fortunate to have an ever-growing variety of sophisticated, off-the-shelf injectable volumizers and fillers that can be used instead of fat.The photo provided suggests that the longish appearance of the face can be broken up by accenting the cheeks a bit--not too much, however, so as not to create feminizing high cheekbones. I would use Voluma XC for this purpose in my Upper East Side Manhattan office and Emervel Volume and Stylage XXL in my Israel satellite facility where a far greater number of regulatory agency approved volumizers and fillers are available. The chin and forehead both appear fine in frontal view. To strengthen and masculinize the angle of the jaw and jawline I would suggest the combination of Radiesse and Perlane L in the U.S. and Restylane SQ and Emervel Volume in Israel. It would be wise to seek out a board certified aesthetic physician with extensive experience and expertise in treating men.
