I had a very bad outcome with a Sculptra procedure almost three years ago. I requested my medical records. He could not locate the treatment record or the lot numer for the two vials. Is this typical of doctors to not have this information available to their patients? I know this question is a bit atypical, but I am at a loss to how to resolve my issues without his cooperation. It leads me to suspect that he is hiding something.

Not all physicians may follow this same procedure but it does not necessarily mean that you had a bad treatment if the information was not recorded.

It is my experience that the relative quality of the records maintained varies rather widely. Answer: Lot numbers should be recorded for Sculptra Every vial has a lot number for Sculptra. All records should be available from 3 years ago. This should be given to you after signing a medical release. Maybe you didn't get Sculptra or it wasn't from a legit source.
