I'm 23 years old and my face started to sag/fall off this way around 4 years ago, i tried gaining weight, but despite my best efforts eating 3k+ calories i could never gain weight or gain the fat back in my face. In summer of 2019, i had perfect skin, my face looked good and i wasn't sagging, then a few months after my skin slowly looked like it was about to fall off. Is a facelift my only option? I got fillers under my eyes and in my jaw a few years ago, but it didn't do much.

Unless your eyes are the problem, you look fine. I don't see any sagging, you have relatively appropriate volume for a masculine type face. You are probably seeing more than what is actually there. I recommend considering looking into sculptra so that you can work on preventative building of collagen at thsi point to help maintain.

Four years ago you were teenager.
Now you are an adult.
Reasonable give up on the Peter Pan syndrome and recognize that you are now in adulthood
Your skin is not falling off your face and there’s nothing wrong with you, nor do you need an operation.
If you want to maintain a youthful appearance, then avoid excessive sun exposure and wear sunscreen every day.
You may also want to avoid the weight fluctuations and do what you can to avoid getting obese at any point in your future.
Sunscreen and the avoidance of excessive sun exposure are by far the most important variables, long-term.
They don’t need fillers, you don’t need surgery and your face looks just fine
