I encourage you to reconsider being so far away from your surgeon. A 40 hour flight is extensive and you will be at a risk for developing blood clots so close to surgery. Generally at least 3 weeks is recommended. If you choose to proceed with surgery abroad it is important that you discuss how you will handle follow up visits with your surgeon. I see my patients regularly after surgery to monitor their progress and answer questions. You will also need to discuss how complications will be handled. You may find that other surgeons are reluctant to address issues arising from a surgery they didn't perform.

If you are planning to have surgery away from home it is important to plan enough time for your recovery. You need to know that if you arrive from an airplane travel the best option is to wait some time before you have your surgery done. How much time you should wait will depende on the length of your flight but since you are talking about 40hrs of travel seems best to wait at least for 3 days once you arrive in the place where you are going to have your surgery done plus a minimum of 15 days after the surgery to fly back. You do need to know that staying for just 15days after the surgery is a short time and is best for you to stay for 3 weeks or more after the surgery. And if any complication arises need to understand your doctor will probably ask for you to stay more time away from home.
Regarding situations such as implants or suture rupture due to airplane pressure the risk for these are in the range of very low to none. Always remember once you have your surgery done the more time you spend with your doctor the better.
