The "best fit" is not based on one single measurement. I would suggest meeting with a board certified plastic surgeon, being examined and measured, and trying on the range of sizes that best fits your frame and tissue. Then, you can actually see what you like.

When implants are placed they will settle behind the nipple at the level of the breast fold. Essentially they will look similar to what you have now, only larger. Your breasts are somewhat wide set and will remain so unless specifically surgically addressed.

An examination is needed to be certain, but your photos do suggest that you do have some lateral displacement. Wearing underwire bras can be helpful, and wearing a good support bra when exercising is needed. If this displacement bothers you, you can have this fixed surgically.

Assuming that you've noticed no change in the size, this could be an early capsular contracture and/or signs of a rare early rupture of a gel. I would consider imaging such as an MRI for further evaluation. Hope this helps!
