The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, fat transfers or small implants can be placed totally submuscular.

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

I recommend an in person consultation with a plastic surgeon to discuss your goals and be measured. After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants, and they choose what looks right for them. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. When implants are placed they will settle behind the nipple at the level of the breast fold. Essentially they will look similar to what you have now, only larger. Your breasts are somewhat wide set and will remain so unless specifically surgically addressed.

While you may benefit from fat transfer in the future, the most definitive volume enhancement would come from implants. Consider implants that might not be as projected, but are generally wider at the base to try to help fill in the space. The downside is that this will give you more volume laterally (side boob) as well. A full consultation would be necessary and I recommend you work with a Board Certified Plastic Surgeon.

Just based on your one photo, implants would look very nice on you. You do not need to do fat transfers, nor will a fat transfer change the location of your nipples, but just give fullness.

Thanks for the question. There is no reason why breast implants should not look great. I do not see how fat grafting will improve your results. Make sure you choose a board certified doc that specializes in breast augmentaiton.

First of all, I would say that you look lovely naturally. In a Victoria Secret Bra, you would likely fit to a C as they make smaller cups with larger letters than others. Putting in an implant will likely push your Nipple areolar complex further out from where they are as the implants will center more and that will push you out to the side. Be careful not to screw up a good thing and consider 3D imaging before you get your implants.

Thank you for your question. First off, you do not need fat transfer or any other “reconstructive procedure.” A few chest and breast measurements need to be made. There is a wide range of silicone breast implant, and breast implant sizes, styles and levels of cohesiveness to the implants themselves. An experienced plastic surgeon can help you thru all of the various options to reduce the appearance of wide set breasts.

All of the suggestions you have heard are viable. I agree with the breast augmentation with round smooth implants. I would go with a moderate + (not high profile or any other higher profile implants as they are less wide and will only cause your nipples to point more lateral)
The use of fat transfer (if required) may be delayed until after your final augmentation results.
