BBL or implants after the first BBL?

How can the fat be preserved after a BBL? How can you look flatter after a BBL than you did before the BBL? Is it better to get another BBL or implants?

Since you did not present any before and after photos it would be very difficult to answer your question. In-person consultation with at least 2 board-certified plastic surgeons with known expertise in BBL (you may need to travel) to see what kind of procedure would best correct your current condition. Allow me to answer your question as best i can without any photos. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made.

You may be in good company with many others who had high hopes for BBL but instead received disappointing results (lumpiness, unevenness, lack of projection in the central and/or lower buttock, insignificant hip augmentation, post-deflation sagging, fatty necrotic cyst or scar tissue, etc.) as most of the fat melts away. It is not quite as much the quality of the doctor nor liposuction technique but rather the quality of the procedure that is a large part of the problem.  Thus, going back for additional rounds of BBL most often produces the same disappointing results but at another financial loss.   Beware, if a surgeon asks you to purposely gain weight (i.e. fat) before BBL, know that the fat you lose first as you get back to your baseline weight after surgery is that very same fat that was transferred into your buttock, hence your new buttock shrinks first, the fastest, and the most...so please do not fall victim to this recommendation.

Fillers, such as Sculptra and hyaluronic acids,  for augmentation of the buttock/hips are a rip-off, period!

Buttock/hip implants, however, provide a more reliable, predictable, relatively safe, and permanent result, especially for adding projection to the hips and central buttock, and often help smoothing out lumpiness. Occasionally a lower buttock tuck may be advantageous for lower buttock fold asymmetries and/or sagging.  When considering buttock implants make sure that the surgeon is going to be placing the implants within gluteus muscles , (a.k.a. “intramuscular pocket”), and that they are using anatomically correct, oval-shaped implants, such as Stanton Anatomic ®️ implants. 
Please keep in mind that an extremely few number of board-certified plastic surgeons specialize in these buttock implants so be sure to consult with one that is reputable and experienced. Unfortunately you may have to travel long and far away from home to find this particular surgeon. Glad to help.

Delivering consistent quality, liposuction and fat transfer results is more difficult than most people realize. The number of plastic surgeons to truly mastered these procedures are in a small minority.
Likewise, not everybody seeking a BBL procedure is a good candidate for the operation.
To make an assessment regarding the outcome of a plastic surgery procedure we generally always need to see a complete set of proper before and after pictures. If you don’t have proper before, and after pictures and ask your surgeon to forward the pictures they took.
In the hands of a sufficiently talented provider, patient should receive an assessment during an in person consultation, making it clear exactly what type of results they are likely to get. And Experian provider should have no difficulty showing you the before and after pictures of enough patients with similar body characteristics to your own to make it clear what your results are likely to look like.
Recognize that early fat transfer results can look very impressive. Represent final long-term results. When reviewing before, and after pictures, always confirm the timeframe of when were taken in a relationship to the procedure. After pictures should never be taken less than 3 to 6 months from the date of surgery in order to be representative.
Never assume that after pictures represent long-term outcomes. Always confirm the timeframe of when after pictures were taken.
Most plastic surgeons in the United States, do not offer gluteal implant augmentation.
There’s a good reason to my moms plastic surgeons don’t perform procedure.
Gluteal impaired augmentation, has a very high complication rate and even higher undesirable side effects, and overall low long term patient satisfaction. 
Patients considering getting gluteal implants should have a clear understanding of what they are signing up for, including all the complications and drawbacks that go with having his procedure.
To minimize complications, undesirable side effects, and surgery failure patients, considering this procedure, should seek out one of the few plastic surgeons who focus their careers exclusively on this type of surgery.
