I already had a extended tummy tuck two years ago with 360 lipo and breast augmentation. I lost over 50 kgs and now I want a tight and bigger butt. I‘m really unsettled and sad because I‘m so unhappy with my butt and every doctor had a different opinion. Please help me.

Thank you for your question and photos. In spite of your past 360-degree liposuction, you still present with a rectangular body shape, alongside a significant amount of fat on your back and abdomen. Regarding your buttocks, I see two distinct structural issues: A square shape with posterior/lateral deflation. Significant cellulite across both buttocks. In my practice, I employ the GoldIncision cellulite treatment first. This releases the deep adhesions from the fibrous bands while applying a subdermal biostimulator. Without this step, your overall buttock shape will not improve significantly. This typically takes 1–2 treatments under local anesthesia. Considering your buttock laxity and ptosis, best option would be a buttock lift. This may include augmentation with  tissues which would be normally discarded consisting of the skin and fat, well vascularized, which will provide significant filler in the upper and central third of the butt. Additional augmentation can be achieved with fat grafts harvested from flanks, back and inner thighs. A scar is placed just above the buttocks and is well hidden with any type of underwear.
