Starting HRT several months before the BBL would provide you and your board certified plastic surgeon valuable insights as to where the fat is being distributed and in what amount. You would not want to perform the BBL before HRT as you may harvest fat from one area that you would later need to replace for contouring.

To me personally it makes more sense to start the hormone therapy first especially if this is for the purpose of gender change.See how much feminization(i’m assuming) you you get from the hormones. At least the way I was educated on managing transgender patients was to have the patients engage with all the non-surgical aspects of gender change before surgical changes.Once upon a time we actually had criteria patients needed to meet for each step along the way.Hormones are going to change the fat distribution on the native fat cells to some degree but I don’t think it’s going to drastically change the outcome of surgery or needing the surgery to be done differently.If you let your body evolve with hormones before the surgery then you don’t have to worry about this issue.Best, Mats Hagstrom MD
