Dear Batavia6074:The molecule of Botox is not known to directly enhance survival of fat grafts. As you point out, in some areas where Botox relaxes wrinkling, there is a synergistic effect with Botox and filler lasting longer together than either one by themselves. This seems to be a mechanical relationship and not a physiologic one. It takes 6-12 weeks to observe how much volume survives from a fat transfer. If there is a large amount of mobility in the area, Botox may enhance survival by reducing a mechanical injury from the overuse of facial expression in that area but this is theoretical at this time without studies. There is a drawback to consider. That is if Botox is used in an area like the cheek, the trade-off will be inability to lift the upper lip, eyelid droop and possibly drooling or food retention in the cheeks from lack of cheek muscle use.
