Ive been considering getting a perineoplasty and Ive been looking at a lot of before and after photos. Ive noticed that in many of the after photos the vaginal opening is no longer gaping or visible at all anymore but in some of the other photos some women still had a little bit of gape. Is this something that cant be helped or is it personal preference or maybe the quality of skills a doctor has? I would be very disappointed to have this surgery and still be left with a slightly gaping vagina
Answer: Considering perineoplasty but have some questions Dear concerned patient. you are correct when one goes for perineoplasty, they should not have any posterior gaping vagina. it all depends on the patient's anatomy post-trauma of delivery and the status of the sphincter. please address your concerns to your surgeon. Answer: It is a combination of many factors Great question! The quality of the outcome shouldn't be based on the post op appearance alone. The goal in performing a vaginoplasty with perineoplasty isn't only to eliminate the gaping of the vagina, but it is also to restore the pelvic floor musculature while optimizing sexual function and pleasure. Many women will always have a slight gaping to the vagina because of the underlying trauma that resulted from giving birth. I see many women for revision vaginoplasty/perineoplasty because they still have looseness and all the associated symptoms, but their appearance may be perfect witout any gaping. These women will often have pain at the opening but feel nothing internally. This is usually the result of a superficial perineoplasty performed to improve appearance, but minimal attention was placed at the remainder of the vagina. This situaton may be a result of surgeon's technique or skill level, but also not the surgeons fault since the birth trauma may be so extensive. You need to have realistic expectations going into this type of surgery, like all other surgeries, but also do your research in identifying the most experienced and skilled surgeon in your area. Some photos may be seen a vaginal gapping is depending on the position of the woman and also the degree of the perineal muscles and skin closure. I would recommend 2 fingers size gap as other wise it will be painful during sex. functionality is very important at this region and keeping the physiological levels should give women more pleasure, prevent pain.

The whole point of perineoplasty is to fix gaping. It will NOT however, make the vaginal canal tighter. For that to happen, you need to tighten the entire vaginal canal and that procedure is called a vaginoplasty.
