I had revision surgery about a year ago. I had 180cc low profile round textured implants and I loved them except for my nipples being placed too high. My PS chose to use 300cc textured high profile implants this time (even though I wanted smooth/mod profile over textured/high profile, I went with what my PS says it's right for me) I gave it a year to see the final result but I still think they look too high and far apart. Is it my anatomy and not something that the surgery will fix it?
Answer: Do my nipples look too far apart or placed too high? Thank you for the question and pictures. Sometimes, whether or not breast implants are in appropriate position can be a matter of opinion or preference. Having said that, if the distance from your areolar to inframammary fold area seems too long (bottoming out) or you wish to have more upper pole volume, then revisionary breast surgery, capsulorraphy (adjustment of the breast implant capsules in the inframammary fold area) techniques may be helpful. If you feel that your nipples/areola complexes are "too far apart", adjustment of the breast implant capsules medially may be helpful when it comes to improving overall breast aesthetics. Medial capsulorraphy would serve to better center each nipple/areoala complex on each breast mound; the potential "downside" with this maneuver will be that there will be more space between your breasts (wider cleavage).Best to communicate any specific concerns/goals directly with your plastic surgeon; in my practice this communication is best done using goal photographs and computer imaging. Generally speaking, the best online advice I can give to ladies who are considering revisionary breast augmentation surgery is: 1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. Ask to see lots of examples of his/her work. 2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals. In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural” or "C or D cup" etc means different things to different people and therefore prove unhelpful. Also, as you know, cup size varies depending on who makes the bra; therefore, discussing desired cup size may also be inaccurate. 3. Once you feel you have communicated your goals clearly, allow your chosen plastic surgeon to work with you in determining the best plan to achieve your goals. Answer: Implant position and nipple position are separate Implants need to be centered behind the mound of the breast tissue. If the pre-existing nipple position is high and/or low relative to the mound of the breast, implants do not change that. If your implants were moved higher to have them more centered behind the nipples, the implants would go almost all the way to your collar bone, which would look very odd. Your results are good, and your nipple position is well within a range of normal.

Answer: Your nipples are far apart but that is your anatomy... but the asymmetry you see can be helped with a revision. If you want your nipples more central, your implants will have to be positioned more lateral which makes your distance between the mounds greater. So cleavage or nipples centered? As for the implant, I always have my patients choose after being fully informed as that avoids conflicts later if things are not as expected so I'm surprised you were 'overruled' by your surgeon.
