I've had a couple of consultation and one surgeon said that I am NOT a candidate for a Brazilian butt* lift or a tummy tuck the other two said I'm a great candidate. One surgeon specifically said he would transfer 400 cc's and insert that to my butt but I'm not sure if that's enough fat being that some of the fat cells die out

Thank you for your photo. In your case this is a slippery slope. The best answer you are going to get would be from a direct consult with a surgeon who has tremendous experience. More photos would help. My approach focuses on gluteal shaping and body contouring not just stuffing fat in your buttocks as is the approach would be for "big booty" surgery. If you are a candidate you could have liposuction and a tummy tuck to contour your abdomen and waist this would make your buttocks look better. The amount of fat from the liposuction can be transferred to selected areas of your buttocks to enhancement. Gluteal sculpting/body contouring is not a cooke cutter approach.

You must have sufficient donor fat on your body to be considered a good candidate for a fat transfer to the buttock. The amount of fat transferred is not as important as the amount of fat that survives the transfer. If you are on the thinner side, you may need to gain some weight prior to surgery. You may want to consult with one more plastic surgeon who is experienced with this procedure before making your decision.

Answer: Injection of 400 cc per buttock is a substantial volume for a thin patient. More may not be possible or even desirable.
If you want more fullness of your buttocks, then fat injection is the answer (which is what a BBL is). The donor fat would be obtained from liposuction of your tummy and hips (or any other area where you have unwanted fat). The combination of liposuction of the midsection and fat injection of the buttocks produces the desired hourglass shape. A volume of 400 cc per buttock is actually a large volume for a patient like you, who has a limited amount of excess fat in other areas. Some surgeons quote larger amounts, but these volumes may contain fluid, not just the fat, so be careful when considering volumes. Also, even if you had more than 400 cc of fat injected per buttock, the fat take may decrease because the injected fat needs to find a blood supply. Injecting too much at one time can jeopardize the blood supply and lead to more fat absorption. So it does not automatically follow that more fat equals better results. I believe the surgeon who indicated 400 cc is being realistic (and even that volume may be a challenge). I've attached a link to my website in case you wish to read more.
