In general, it is best to do your homework and seek the advice of a board certified rhinoplasty specialist. Rhinoplasty is a very complex surgery, and requires the skill of an experienced surgeon that has performed a great amount of procedures. When looking for a surgeon, consider credentials, experience, before and after photos that showcase the surgeon's results, fantastic patient testimonials, and the surgeon's office. During a consult, the surgeon may use computer imaging software, which will allow them to morph a photo of you and give you an idea of how the final result of surgery may appear. It is important that you find a specialist facial plastic surgeon that you trust. Tell your surgeon what you want during your consultation including what to do in the case of a poor rhinoplasty. Some surgeons also offer computer imaging to help their patients envision the post-surgical result. Overall, good communication will result in a more successful rhinoplasty.

From there, you can look at before and after photos of the selected surgeon and find a nose that is similar to yours

Finally, meet your prospective surgeons and go with your gut feeling on who is right for you

In selecting a rhinoplasty surgeon, you must see his before and after photos, of noses of your type. You need to see that he has succeeded in doing for someone else, what you want for yourself. If you post your photos, I can give you an idea of the types of techniques needed, what to look out for, what to be certain you communicate to your surgeon.

How to avoid a bad rhinoplasty is an excellent question. Forget looking at imaging and make your decision based on the experience of the surgeon and does he do natural noses. Meet with one of his patients that has a nose similar to yours and is one year post operative and you will have an idea whether or not this doctor has the experience to give you what you want-a natural looking nose.

Unfortunately, we don't have any photos of your nose to evaluate. This is the most important missing piece of information. I recommend that you consult with several rhinoplasty specialists. They will evaluate your nose. Be very clear about what changes you want to make to your nose. You can also bring photos of results that you like as well as results you dislike. It is actually very helpful to know what type of look you want to avoid. You can also request that your surgeon perform video-imaging to simulate the desired results prior to surgery. This will ensure that you and your surgeon have the same goals for your nose. I hope this information is helpful.

You haven't included photops of yourself, and you haven't described what you specifically dislike about the results in the woman's rhinoplasty whose nose and face shape is "almost identical" to yours.

Your goal illustrations aren't even photographs, but clearly show residual dorsal hump in both, to differing degrees, and one diagrams residual overprojection. But it's "over" only if that is what you are trying to change.

See one or more experienced ABPS-certified and ASAPS-member plastic surgeons who do lots of noses and have been in practice for a couple of decades, and tell him/her exactly what it is you like and don't like about your nose.

Although the patient in the first photograph is not as "model perfect" as many goal photographs that are brought into our offices, her result may be exactly what she wanted, and most plastic surgeons would characterize this as a good result. But ONLY if that is what the patient requested! The depicted patient could be absolutely thrilled with that result, but it is clear that you would NOT be. That's OK, but it does not make the result shown an "awful" one, only an outcome you do not want!

You, presumably, would be requesting more residual projection, a tiny bit of hump remaining, and really very little change.

But why? To keep people from knowing what you've done? Or to placate a parent or spouse who thinks you "shouldn't have anything done" for one of several reasons? (Aren't you proud of your heritage?) (Not good enough to look like the rest of the Joneses or Smiths or Johansens?) etc. This should be for you, not anyone else. And if a specific result is what you prefer, for the right reasons, then that's perfectly OK!

Show your consulting surgeons your actual nose, discuss your specific goals, and have realistic expectations. I suspect that in the right hands you can have a wonderful result . . . if you can effectively communicate your goals and have the proper motivation! Best wishes! Dr. Tholen
