How to revise Blanch spot with bony lump on nose tip that appeared 8 months after nasal cartilage implant on the tip?

I had hump removal, dorsum narrowed (to prevent open rooftop), deviated nose straighten and a Nasal cartilage grafted on nose Tip. Prob: Blanch spot with hard bony bump on nose tip appeared since 2 mths ago. I believe it's a protuded grafted-cartilage sticking out underneath my Thin Skin (Shrink wrapping?). 1. Is it possible to fix this blanch spot without the need of fascia implant? 2. How to smooth this protruded cartilage without open rhino? Any suggestion will be appreciated. Thank you.

It is possible that there is too much pressure on the skin by the deeper structures. This usually subsides as the skin relaxants.

Thank you for your question.  Thin skin can definitely make any minor irregularity or imperfection show.  You have a few options.  Judicious use of filler may soften the edges if the graft looks good, but its edges are a little too apparent.  A fascia graft is a very good choice for a more permanent solution and can often be accomplished under local anesthesia.  This can usually be done without an external/open approach by an experienced rhinoplasty surgeon.  Smoothing the edges may be possible without a graft, but photos or an examination would be necessary to know if this would be feasible.  See your surgeon and discuss your concerns so they can hopefully address them to your satisfaction.  All the best!
