Best lasting option for jowls, loose jawline and under-chin laxity at 39? (Photos)

I’m 39 and will be 40 in September. I finished chemotherapy three years ago and have also gained weight since then, so I know some facial fullness may be weight-related. However, my main concern is that the lower part of my face and neck look noticeably looser and more “melted” than they used to. I dislike my jowls, softened jawline, marionette area and the fullness/loose skin under my chin. I also have dimpling near the corners of my mouth when I contract my lower-face muscles. I have already tried Sculptra, cheek filler, filler in the marionette area and Radiesse, but I do not want to keep paying for temporary treatments that do not really correct the laxity. My goal is a more defined jawline, smoother lower face and tighter neck with the most durable result possible. I would prefer one effective procedure over repeatedly spending money on threads, filler, lasers or treatments that only give a subtle temporary change. Based on my photos, does this look mostly like fat, loose skin, muscle laxity or tissue descent? Would FaceTite or another tightening procedure realistically help, or would a lower facelift, neck lift, chin lipo or combination make more sense? I am open to surgery if that would give the best lasting result, but I do not want to over-treat something that could be corrected more simply
