You should wait at least 2 weeks before sitting without a support cushion and sleeping on your back. Sitting with no support too soon can harm the newly moved fat cells.

We advise our patients to avoid sitting longer then 30 min for approximately 2 months after fat transfer to buttocks. Long lasting pressure over fat grafted area will interfere with take of fat grafts and diminish final volume and result of augmentation. Using inflatable pool tube in the car, home, work is an affordable way to avoid pressure over grafted area. Good luck.
Every surgeon has a different recovery protocol so it is best to talk with your plastic surgeon about what he/she recommends for you. With that said, I advise my patients not to sit on their rear ends at all for the first week, this includes driving. After then they may sit for short periods of time with a pillow under your legs above the knee for 8 weeks, it doesn't have to be a special BBL pillow if you don't want to draw attention to it. I would not recommend sitting normally without a pillow for 8 weeks. Dear greeneyes42,I instruct my patients not to sit for 2 solid weeks. We provide them a specialized pillow that our patients can sit on during that time. They can sleep on their stomachs or sides. Fat viability depends a lot of surgeon technique. The fat has to be placed back into the buttocks as soon as possible so that it has a greater chance of survival. Great question! Generally once you are off pain medication then you can start driving. However it is important to always use your BBL pillow while driving as you do not want to actually be sitting on your buttocks for at least 4-6 weeks. I always advocate following the post-operative protocols of the patient's PS as protocols vary from PS to PS. Moreover, your PS is the most knowledgeable of your surgery and state of recovery and is ultimately responsible for your best results.There are no hard and fast rules on when to drive or for what distance after a BBL once you are no longer taking pain medication. Most plastic surgeons would recommend limiting your driving the first 6 to 8 weeks following the BBL. I recommend that my patients minimize sitting, even with a pillow, for the first two weeks. If they have to sit, it is critical that every 30 minutes or an hour that patients get up and stand for a few seconds to take the pressure off. This reduces the chance for fat necrosis. After 6 weeks the fat has revascularized, and you have a very good idea of your final results. Different recovery protocols are present for different patients. On average patients may drive as early as 2 weeks and as late as 6-8 weeks; meaning your recovery depends on the amount of fat removed from liposuction, the areas liposuctioned, and the amount of fat transferred to your buttock. A BBL pillow is also advised during driving or sitting.
