In most cases hormone-based contraceptives do not need to be stopped or removed in anticipation of surgery and the same precautions to avoid blood clot formation should be followed - limiting operative time, leg compression devices, possible anticoagulation, early ambulation, staying hydrated, etc. Talk to your surgeon, they can better provide you with reassurance or recommendations based on your specific medical history and any associated risk factors such as smoking. The Mirena uses progestin-only, which carries a lower clotting risk than estrogen-progestin combination therapy. Your surgeon should evaluate your overall risk of DVT and PE, including the presence of the Mirena. Your surgeon may recommend certain anti-clotting therapy after surgery if your risk is higher than average.
