The amount of buccal fat pad to be removed during buccal at removal can be customized to both patient's goals and patient's anatomy. Removing all buccal fat removal does NOT fit all the patient's profiles. In my buccal fat removal practice, I do have patients that get either small amount, moderate amount or all the buccal fat pad removed. This is decided both during consultation and during surgery. Therefore, if you are interested in a partial buccal fat removal, a plastic surgeon specialized in buccal fat removal should be able to accommodate your wishes. Based on this single photo it looks that you have some atrophy of midface (flat, sunken area underneath the lower eyelids!) due to a small cheek bones (malar hypoplasia)? For that reason, your lower cheeks look bigger? I would rather consider cheek augmentation (fillers, implants) than buccal fat removal.
