I am a facial feminization surgery (FFS) specialist, and although I have not seen this occur in my patients, there are several possible explanations for what you’re describing.It could represent a small cyst or a bony overgrowth. If titanium plates were used to secure the anterior table of the frontal sinus during the setback, one of them may have migrated or become palpable over time.If resorbable plates were used, they can occasionally cause a sterile abscess as they begin to dissolve — this typically occurs around 12 months after surgery.The area you described also corresponds to the trajectory of the supraorbital nerve, and given the associated sensitivity, a supraorbital nerve neuroma is possible (though rare). Alternatively, the lesion itself may be compressing the nerve, which could explain the discomfort or pain.I recommend seeing a specialist experienced in facial plastic surgery who can perform an appropriate evaluation. An ultrasound may be helpful as an initial step, but a CT scan of the maxillofacial region (with and without contrast) might be necessary for a definitive diagnosis.
