Liposuction is the preferred treatment for Dercum's disease, if this is possible.Sometimes excision is necessary to get rid of large symptomatic areas where liposuction would leave a large amount of excessive skin.
I have treated several patients with Dercum's disease. It can respond to excisional treatment of the most affected areas, however I have found that liposuction of the most symptomatic areas tends to achieve a better success rate, albeit with a moderate possibility of recurrence of some of the symptoms.
