I have treated several patients with Dercum's disease. It can respond to excisional treatment of the most affected areas, however I have found that liposuction of the most symptomatic areas tends to achieve a better success rate, albeit with a moderate possibility of recurrence of some of the symptoms.
I would suggest doing circumferential thigh liposuction. You can expect to have significant post operative swelling but the end result will be much better.
