6 months post op - Donut lift + implants, tuberous breasts drop and fluff (Photos)

Question

Hi, I have had a breast lift and augmentation, 275 cc mentor, exactly 6 months ago. I'm 38, athletic, with strong chest muscles, that's why my surgeon did subfacisal implant for me.My right breast has not fully dropped yet! the inner part has dropped, but the outter part is still high,that unbalanced dropping has caused a mini double bubble saped.Is it a double bubble?Or just unbalanced dropping? Do you think I needed a lollipop lift? Or other plans? Is there still chances of dropping?

I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

Your implants are too large for your body. You can see that your breasts are wider than your chest wall. Also, you have stretch marks on the chest skin. I recommend you undergo another donut mastopexy with smaller implants. You would also benefit from plication of the pockets laterally and inferiorly to reposition the breasts more higher and more medially.

Subfascial placement of implants is a vogue phenomenon that is being applied to everyone. How quickly my colleagues forget that it offers little soft tissue coverage and will likely age just as poorly as many patients with subglandular implants. To your question, a donut lift was a terrible choice by your surgeon and you were never a candidate: persistently wide areola, prominent scars, radial pleating around the areola, persistent sagging of the breast mound, and poor nipple areolar placement on the mound. If you are unhappy, you need to find a great revision breast specialist.

The donut lift would have been the better option. However you will likely have to go with smaller implants as well otherwise the closure of the incisions will be too tight

I would suggest trying oral Accolate and external ultrasound treatments to encourage your capsule to soften and let the implant settle down better. Otherwise, it will take surgery to release that capsule.

Unfortunately, because of your deformity you will eventually need a vertical mastopexy. Even if your implants drop, your nipples will be below the equator of the implant.

More of a concern, rather than the mild double bubble deformity, in my opinion is the shape and continued ptosis of your breasts, and the widening of your areolae.  It is for these reasons that I rarely, if ever, recommend a periareolar, or donut breast lift.  However, the old 6 month waiting time for breast implants to descend is a general number and may not apply to everyone.  So there might still be some implant settling to come, especially in a subfascial setting.  You might consider having to convert to a lollilop lift if that does not happen.  Good luck!
