Do I need a free nipple graft based on my ptosis? (Photos)

I’m going to be honest, I did not expect being told I would need a FNG based on results I’ve seen online. They’ve always been more long than round, I‘m a 34E. I also obviously don’t want to risk nipple necrosis, but I also do not want to lose sensation in my nipples with a FNG. I also mentioned that I’m losing weight and I’m aware that I might just end up needing a lift. However, the surgeon mentioned that with the volume loss after losing weight I’m basically guaranteed to need a FNG. He mentioned that he could try doing a reduction without a FNG right now and when I end up losing weight, we could try doing a non FNG lift again depending on how much the ptosis is then. He did mention a higher risk of losing a nipple though. I was going to go for a couple of more consultations, but if these are my only options, I don’t think I even want to move forward with it. Is a FNG truly my only option?

You do not need a free nipple graft for your lift/reduction. The technique I recommend is the Bellesoma. This will reduce and reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. The weight of the breast is transferred to the underlying pectoralis major muscle resulting in pain relief without excessive reduction and long term stability. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained.

You do not need a free nipple graft. Get your surgery performed by a expert breast surgeon and you'll likely get a great result.

Short answer is no, I would not plan on a free nipple graft in your case.  Each surgeon has their own technique and their own past experiences and they will make their recommendations to you based on that.

It is a possibility that if, during  surgery, your surgeon decides the blood flow to the nipples is inadequate he or she may do a free nipple graft so you mmight want to clarify that, and get a few opinions as well from other board-certified plastic surgeons. Cheers from Texas !

First, I would like to mention that these photos are not ideal for an accurate assessment. If possible, photos taken by someone else from the front, clearly showing both breasts, would allow for a much more reliable evaluation.
Based on the photos you have provided, however, my preliminary impression is that I do not think a free nipple graft would be necessary. I believe that an excellent outcome could likely be achieved using a Wise pattern breast reduction technique.Please keep in mind that this is only a preliminary assessment based on the available photographs. If you are able to send higher-quality photos taken from more appropriate angles, we will be able to provide you with a more accurate opinion regarding the most suitable surgical technique for your case.
