My goal: a better profile, more facial balance and an improved neck/chin angle. My concern: I do not want to have visible scarring in front of my ears or hairline. Therefore, I’m considering a chin implant and a submentoplasty that includes muscle tightening (platysmaplasty), which only involves an incision under the chin. Is this a good option for me? With respect to your neck, you appear to have some fat deposits in your neck, which are located both above and below the platysma muscle. A neck lift procedure through a 1 inch submental incision can accomplish removal of both compartments of fat. Liposuction is performed to remove the fat deposits above the muscle while surgical extraction of the fat deposits below the muscle is performed, and then a platysma plasty will significantly improve your jawline. Both the neck lift, and the chin implant can be performed through a 1 inch incision underneath the chin with no external visible scars in front or behind the ears. The skin laxity that you have in your neck cannot be addressed without placement of those incisions in front and behind your ears at age 52.
