This pleasant woman presented with the goals of reducing her nostrils. Approximately 3-4 mm of nostril was removed on each side. This was performed in clinic and was well-tolerated. Her scars are unnoticeable.

Nostril widening using internal flaps following over-resection performed elsewhere.

Patient had a previous rhinoplasty with an aggressive alar base narrowing. The after photo shows reversal of the narrowing using a modified liplift procedure.

This is a healthy male who presented with a history of two prior rhinoplasties. He was bothered by the significant hump noted on his profile in addition to the width of his nostrils. After evaluation and discussion in clinic, it was determined that the hump appeared after placement of Gore-Tex under the bridge for camouflage during the revision procedure. The skin started to appear discolored and tender. He underwent an open rhinoplasty procedure with us to remove the foreign material, smooth the bridge and cover it with fascia from his temple, and narrow the width of the nostrils. Gore-Tex, medical-grade material is often used to hide irregularities underneath the skin of the nose, but it can produce poor results. As a result, we use temporalis fascia which is a natural blanket that comes from the patient's temple region.

45-54 year old man treated with Alarplasty

This is a healthy patient who presented with the request of narrowing her nostrils. The recommendation was made for an alar base reduction. This involves an incision on the underside of the nostril which, when done correctly, heals very well. The width was reduced about 3.5 mm on each side to create more narrowness. This can be done in-office under local anesthetic and with minimal swelling, bruising, or pain. The important point is to avoid too much narrowness which can make the nose appear operated or unsightly. The patient is pleased with her natural result.

Patient had nasal base overly narrowed during a rhinoplasty performed one year prior to my seeing him. I lateralized the ala nasi using composite ear grafts.
