This woman was dissatisfied with her small breast size and lack of fullness. She was interested in a noticeable increase in size, but a very natural appearance. In particular, she did not want to see excessive bulging in the upper breast, or obvious signs of the implants. She was treated with round, cohesive gel implants, 295g in a dual-plane position.

Our patient wanted her breasts to look lifted, not just larger, and she chose Dr. Timothy Katzen to help refine both shape and position in one surgery. She is a 65-year-old White female who weighed 115 lbs at the time of her procedure. Dr. Katzen recommended a breast implant with a breast lift because implants add volume, whereas a lift repositions the breast tissue and nipple to a more youthful height, improving balance in bras and swimwear. Under general anesthesia, he carefully measured and marked the breasts to plan how much skin should be removed and where the new nipple position would look most natural. After creating the lift, Dr. Katzen placed the implants to achieve upper fullness and improve projection, then reshaped the breast tissue over the implant so that the contour appears smooth rather than round and unnatural. The incisions were closed in layers to promote healing and minimize scarring, and post-operative support garments are typically used to reduce swelling and protect the new shape. These photos were taken 2.5 months after surgery, and her results are already remarkable, with improved symmetry, a more prominent profile, and a more confident silhouette that continues to refine as swelling subsides. Dr. Katzen creates a tailored implant pocket, checks symmetry with the patient upright, and controls bleeding before closure. Aftercare usually includes a supportive bra, light walking, and scar care to protect the results.

A softer, more natural breast shape is often possible after implant revision surgery when painful capsular contracture is properly treated. Our patient is a 44-year-old Asian female who previously had bilateral breast augmentation and later developed bilateral painful breasts because of capsular contracture around her saline implants. She came to Dr. J. Timothy Katzen seeking the removal of the bilateral saline breast implants, bilateral capsulectomies, and replacing them with silicone implants. Under general anesthesia, Dr. Katzen performed this breast revision procedure by removing the old implants, surgically excising the scar tissue capsules surrounding them, and placing new silicone breast implants to improve comfort, contour, and softness. With breast implant surgeries, capsular contracture happens when the natural scar tissue around an implant becomes tight and constricting, which can cause pain, firmness, and distortion. A capsulectomy is performed to remove that problematic scar tissue so the breasts can be reconstructed more healthily. Silicone implants were chosen to provide a smoother, more natural-looking, and natural-feeling result. Her photos were taken 1 month after surgery and already show remarkable early improvement in breast shape, symmetry, and softness. She is also relieved of most of her pain, making her outcome a beautiful example of how revision breast augmentation can improve both function and appearance.

Breast Augmentation & Capsulectomy to Treat Painful Breast Capsular Contracture

Removal and replacement of breast implants with capsulectomy. This patient came to see us after multiple failed attempts to fix her breasts. She had a painful and distorting Baker IV capsular contracture which had recurred with every attempt and fixing this difficult problem. I performed a unique procedure where I used an acellular dermal matrix (ACDM)/Strattice to act as an spacer to prevent recurrence of the capsule. Without the ACDM the capsule is very likely to recur. In addition I used a new Motive Silk Surface implant which has the lowest incidence of capsular contracture. Finally I placed her on Singular which also prevents capsular contracture. She not only looks better but feels much better too.

Breast implant revision can help patients who are unhappy with the shape, position, or overall appearance of older implants. Our patient is a 40-year-old White female who came to Dr. Katzen because she did not like how her breast implants looked and wanted them replaced with new implants. Dr. Katzen recommended breast augmentation combined with a breast lift to improve both volume and position. She was placed under general anesthesia for surgery. During the procedure, Dr. Katzen removed her existing implants and replaced them with new breast implants. He also performed a breast lift to raise and reshape the breast tissue, improve nipple position, and create a more youthful breast contour. Breast augmentation with lift surgery is often recommended when implants alone cannot correct drooping, stretched skin, or a low breast position. This combination helps refresh the breasts by improving fullness, shape, and support simultaneously. Her photos were taken 4 years after surgery, and our patient achieved remarkable results. Most importantly, she feels more confident in her new body. This case shows how breast implant replacement, breast lift surgery, and breast revision can work together to create a smoother, lifted, and more proportional result. Dr. Katzen customized the surgery to address her implant concerns while improving the overall shape of her breasts. The goal was a refreshed breast contour that looked balanced and proportional to her body.
