Dr. Ram Kalus is a board-certified plastic surgeon in Mount Pleasant, South Carolina, and Tel Aviv, Israel, with over 32 years in practice. He specializes in aesthetic and reconstructive surgery of the breast, body, and face, with a focus on breast enhancement procedures—including breast augmentation, mastopexy, breast reduction, and immediate and delayed breast reconstruction, along with revision breast reconstructions that may have been performed elsewhere in the past—as well as tummy tucks, labiaplasty, and complete mommy makeovers. The founder of Plastic Surgery of the Carolinas, as well as his practice in Tel Aviv in 2017, Dr. Kalus takes pride in spending as much time as necessary with his patients to answer all of their questions, so they are confident about their surgical journey, and to perform the best possible operation for them. “Much of the anxiety surrounding plastic surgery is the unknown, and fear of a bad outcome,” he says. “We try to dispel and address both concerns for every patient.” Dr. Kalus’s decision to become a plastic surgeon was due to his lifelong appreciation of aesthetics and the physical skill that’s required to create a beautiful outcome. “When I discovered my love of working with my hands and began to understand the power of reconstructive and aesthetic surgery, I became more and more drawn to the specialty,” explains Dr. Kalus. He is also inspired by the influence a person’s confidence in their physical appearance can have on their life. “The extent to which we can positively impact a patient’s self image with our knowledge and skill is both humbling and gratifying,” he says. Although he specializes in aesthetic surgery of the breast and face, Dr. Kalus makes no distinction between aesthetic and reconstructive surgery. “A breast reconstruction is no less an aesthetic operation than a breast augmentation. A reconstruction of a nose or forehead defect is no less an aesthetic procedure than a rhinoplasty or blepharoplasty,” he says. “My absolute cardinal principle is to leave the patient appearing as though they never had surgery.” Dr. Kalus also enjoys performing aesthetic facial reconstruction, as it respects the complex topography of the face as much as the anatomy, and makes every effort to leave the patient with an “unoperated” appearance. He is also known for his expertise in both primary and secondary labiaplasty. “This procedure is less frequently discussed, yet the need for which is clearly underserved from the standpoint of women suffering needlessly from labia minora hypertrophy, or disfigurement from improperly performed primary procedure,” explains Dr. Kalus. “This can be a source of significant improvement of quality of life for countless patients.” After earning his medical degree from Boston University, Dr. Kalus completed his general surgery training at Hartford Hospital and the University of Connecticut. He then underwent plastic surgery training at the Dartmouth-Hitchcock Medical Center in New Hampshire, the University of Massachusetts Medical Center in Worcester, Massachusetts, and in Paris, France at the Hospital for Sick Children. Board certified by the American Board of Plastic Surgery, Dr. Kalus has been an active member of several prestigious medical societies. He is a fellow of the American College of Surgeons and an active member of the American Society of Plastic Surgeons and the American Society for Aesthetic Plastic Surgery. A: Thank you for your question and photos. An examination in person is required to determine how to best advise you. That said, based on your photos, it appears that you would be an excellent candidate for a subfascial breast augmentation and might be able to hopefully avoid a mastopexy and its attendant additional scar burden. Please consult with an experienced board certified plastic surgeon whose aesthetics seem to align with your own based on his or her before and after gallery, and based on recommendations or reviews. A: Thank you for your question. As you undoubtedly know, a precise treatment plan or specific implant recommendation can only be determined with a careful examination and detailed discussion with your plastic surgeon. Volume (300 versus 400), is far less significant than implant width and projection, as you already learned from your surgeon. Implant width is similar to shoe size. The implant literally needs to fit into your breast and chest. The 300 Moderate Plus may well “fit” your breast, but the volume and projection are what will determine the “look” you want, similar to the style or type of shoe you would select. It is true that for a given volume the higher the profile or projection of the implant, the narrower the diameter. Other considerations such as saline versus silicone are also important, as are "round" versus "anatomical" or "form stable" or "shaped", smooth surface versus textured, and softer versus firmer with the different levels of gel cohesivity. The term “natural” can mean different things to different people and the profile of the implant does not necessarily determine that. You can certainly achieve a “natural” appearing result with a high profile implant if properly selected, but as a rule, the larger the implant and the higher the profile, and certainly if the implant is positioned too high, the greater the chances you will look “augmented” because of exaggerated upper pole fullness or convexity. In my practice, besides a thorough interview process and physical examination with careful measurements, we use on lay sizers within an unpadded bra to help you understand what you are likely to look like in a bra, bathing suit, or T-shirt with a given size implant. In addition, and this is critical, we ask all of our patients to provide us with photos of their desired final look, with images of women’s breasts that most closely resemble the look they are trying to achieve. This serves as a visual guide that thankfully rarely leads us astray. The most common reason for patient disappointment after breast augmentation is size-too big or not big enough. This algorithm which we have been employing for over thirty years has proven to be very successful. Because all of these factors can leave you quite confused, it is crucial that you select an experienced board certified plastic surgeon whose aesthetics align with your own, and with whom you feel comfortable. Don't be reluctant to seek a second or even third opinion if necessary to be sure and feel confident about moving forward with scheduling surgery. Expressing your desired cup size is notoriously inaccurate, and final breast size is the most common reason for patient disappointment after breast augmentation, either too big or not big enough. I recommend discussing your concerns with your plastic surgeon, who knows you best. A: Thank you for your question and photos. An examination in person would be required to properly evaluate you and provide you with specific recommendations. That said, based on your photos I believe you would be an excellent candidate for implant placement above the pectoral muscle. My own technique is subfascial rather than subglandular. To my judgment you will most likely be able to avoid a lift (mastopexy) with excellent and proper redraping of the breast over the implant without the need for a lift. Do your research and select a board certified plastic surgeon whose results and reviews appeal to you based on his or her gallery and website. Best of luck to you.

A: Thank you for your question and photos. The reason you look the way you do in a bra that brings your breasts (and implants) closer to each other is that the skin and soft tissues over your sternum in the midline are tented similar to a pole that raises a tent. Part of this is related to your anatomy, and it also relates to the diameter, volume, projection and cohesivity (firmness) of your implants. Whether you call it “synmastia” or not is not as important as knowing that you are not entirely happy. I recommend discussing your concerns with your plastic surgeon. If you feel a second opinion is warranted please research a board certified plastic surgeon whose aesthetics align with your own based on his or her Before and After gallery. Best of luck to you.
