Richard J. Bruneteau, MD
Board Certified Plastic Surgeon

Dr. Bruneteau is from Omaha and he attended medical school at the University of Nebraska Medical Center. In addition to his general surgery residency at the University of Iowa he also completed a residency in plastic surgery at the University of California. Dr. Bruneteau finished a fellowship in micro vascular surgery at the University of California and a fellowship in craniofacial surgery at Harvard Medical School. Dr. Bruneteau returned to Omaha in 1990 and has been in practice at ASI since that time. His decision to become a plastic and reconstructive surgeon was influenced by his desire to help people. Plastic surgery allowed him to do this in a way that used artistry and creativity.

Marie E. Montag, MD

Dr. Marie Montag completed her residency in general surgery at Creighton University. She traveled to the University of Wisconsin in Madison to continue training. She studied Craniofacial and Pediatric Plastic surgery for another year under Dr. Louis Morales in Salt Lake City, Utah. She made the decision to become a plastic and cosmetic surgeon because it’s an artistic and creative way to practice medicine. Her dad and brother are both sculptors, and plastic surgery is her creative outlet.

Dr. Edney is from Omaha, Nebraska & graduated from the Creighton University School of Medicine. He completed his five-year residency in general surgery at Creighton University where he received his board certification in general surgery. He also completed a one-year fellowship in surgical oncology at Creighton University. Dr. Edney completed his plastic surgery training at the University of Pittsburgh where he remained for an additional fellowship in micro vascular surgery. After experiencing all the different specialties he was attracted to plastic surgery because of the wide range of procedures & the creativity required in many cases.

Dr. Biggerstaff was born and raised in Lincoln. He completed his undergraduate degree from Creighton University, with a bachelor of science in physics. He received his doctorate of medicine degree from the Creighton University School of Medicine in 2012. He finished a five-year residency in general surgery at Creighton University, and then completed his plastic surgery training at the University of California, Irvine in 2020.

Q: 40s, 5'4, 127lbs, are there other repair options beside full tummy tuck? (Photo)

A: Hi ccagle21, I suspect your best result would be with a full tummy tuck with liposuction, which would likely achieve the best overall contour, provide the best skin tightening, and allow improvement of the umbilical area. Because of your high belly button, and the fact that your upper abdominal skin does not have severe laxity, there is a chance that you may require a small vertical scar or 'T' scar of your lower abdomen, but I would typically prioritize a lower transverse abdominoplasty scar and accept a small vertical component if needed (which is majority of times still below the bikini line) versus raising the transverse scar. A: Hello johnfm, occasionally, direct excision of the glandular breast tissue from the area under the areola can leave a deficiency of soft tissue, especially in cases where the mass is relatively. It's not corrected, once healing occurs, and the swelling goes down. You can end up with some nipple depression. The goal is to create a relatively smooth contour by closing multiple layers, and in some cases, this involves some rearrangement of the nearby fat. Based on your photo, you do appear to be a candidate for revision, although an examination would be required to be sure, and also to determine the best method of revision. Likely, the same incision could be used to re-elevate the areola, rearrange some of the fat tissue to fill in the soft tissue deficit. There is a possibility, if the tissues are not too tethered by scar, that you may have some improvement with fat grafting, which would be slightly less than invasive, but in my mind the best result would be revision through the original incision. Q: Can I use a plastic piercing retainer during surgery?

A: Hello bdubs2, each surgeon is different and most are very picky in general, but in my practice (and I would guess the majority of practices) I would be alright with proceeding with a plastic piercing retainer. I would double check with your surgeon to be sure he or she is alright with it, however. That way there are no surprises the morning of surgery. Hope this helps. Thanks, Brian.

Q: Male facelift incision: why risk a scar in front of the ear?

A: Hello Original3835, this is one of the difficult decisions regarding male facelift, because neither option is perfect. You seem to have a good understanding of the trade-offs. Your logic is not incorrect. You can get laser hair removal or electrolysis hair removal over the tragus to make for a more natural hairline. This implies additional cost/time and some discomfort. Effectiveness of laser hair removal is somewhat dependent on your skin/hair pigmentation (works better for darker hair) and also often requires maintenance treatments to keep hair from growing back. Electrolysis has been more effective in my experience, especially in lighter haired individuals.

Q: Which procedure would deliver the best results for targeting the jowls? Previously had a mini facelift and Restylane (photos)

A: Hello whodeewho1, a facelift/necklift lift (either SMAS facelift or deep plane facelift) will be much more effective than fillers at improving your jowls and giving you more definition of your jawline, and will improve the appearance of your neck as well. It is difficult to know what technique or the extent of your initial mini-facelift procedure, but typically with a full facelift (including a SMAS procedure) you would notice immediate improvement in the jowls. Thus, you would likely get the best improvement with a secondary facelift. Hope this is helpful. Thanks, Brian
