Given your good skin tone and mild grade 1 ptosis, gummy bear implants should restore volume and enhance the shape of your breasts without the need for a lift. This would help with the upper fullness and give you a more youthful appearance, although it may not fully address the nipple position or any minimal sagging. If you're comfortable with a slight amount of sag, this could provide a natural-looking result with a reduced recovery time.
However, if you're aiming for a more "perky" appearance and want to address nipple position or skin laxity more effectively, a breast lift combined with the augmentation would likely provide the best outcome. This would reposition the nipple and remove any excess skin, creating a firmer, lifted shape. Dear Sincere167773, thanks for sharing your questions and concerns, after evaluating your case if you are looking just volume breast augmentation with implants will solve this, but due to the position of your nipple under the inframammary fold and this little ptosis you look like a great candidate for breast augmentation with implants rather than lift with this you can ensure long term perky results. This answer can be a difficult call without a good examination. It seems from your photograph that you might do very nicely and have and excellent result without a lift. Good luck with your surgery!

determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.

It’s difficult to give you an opinion without seeing a side photo. If your nipples are located at or below your breast fold, then a lift would be recommended. If not, then no lift is needed.

Your single photo suggests the amount of sagging is mild and a lift would not be necessary. An implant alone should suffice. Seek a consultation with a board-certified plastic surgeon to confirm this.

Breast augmentation without lift should restore fullness. Above muscle or dual plane implants should yield nice results.

Breast will look more natural and still sit a little low overall.

Repeat exam and consultation with your plastic surgeon is recommended to confirm this as well as discuss your options and expectations.

You do not need a breast lift. Implants are an alternative and are not lifetime devices. Another alternative for you to consider, since you are choosing liposuction, is fat transfers. This will give you some upper pole fullness, if that is what you desire. Also, you can undergo a donut mastopexy to pull in some lack skin. If you want implants, I recommend small implants placed submuscularly.

With that history of weight loss it is likely your breast tissue doesn't have good elasticity. If you don't mind suboptimal nipple position on the breast mound then don't get the lift. If you're expecting 'perky' nipple position and minimal tissue hanging below your inframammary fold, you'll need a breast lift. Plain and simple. Any experienced breast surgery expert would tell you this.
