Surgeon will decide CCs on table during surgery. Is this typical?

Thanks for your question! I really like my patients to be involved in the decision-making process. We will try on sizers, look at various B&A photos, etc. If you are concerned or aren't feeling 100% comfortable, call the office and express this concern. Hi, thank you for sharing your question. During a consultation, after measurements are taken, a range of implants are selected that will fit both the patient's and surgeon's criteria. The final decision is made on the table once the pocket dissection is done and tissue pliability/characteristics are understood. I hope you had a successful outcome.

Implant size discussion and implant selection is one of the most important steps during your consultation with plastic surgeon. The decision-making process depends on patient’s desires, as well as breast and soft tissue dimensions. . I also utilize Vectra 3D planning to further help with implant selection.
Typically, silicone breast implants are used because they offer more natural feel than saline (salt water filled) implants and the size/volume is discussed prior to surgery.
I suggest that you seek an in person consultation with a Board-Certified Plastic Surgeon in order to discuss your goals and concerns for the best cosmetic results. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results.

After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants and they choose what looks right for them. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. You get to see actual results and the surgeon's experience and not a virtual idea of what can be achieved. An experienced surgeon knows what size to use in which patient. You should at least have been given a size range. I always use the size implant we select before surgery and make sure the patient and I are in full agreement. Otherwise there could be much finger pointing after surgery. Ask the surgeon to tell you the size. It's not that hard

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

The patient and I decide exactly what size I am going to use in most cases. During the consultation, I have the patient try on a variety of sizes to see what she likes and how it will look. Every woman has her own idea of what "is natural" and what is "too large" and it does not always agree with mine. Sometimes, someone is replacing their implants and does not know the current size. In those cases, I bring a selection of sizes and go as close as I can. I would suggest going back to your surgeon and ask to try on sizes to see what you really like. Your surgeon may do excellent work, but his opinion may not be the same as yours.

hi there, its pretty normal not to agree an absolute size/implant as sometimes on the operating table a bit more volume to lift the breast if its a bit droopy can be helpful, or the tissues are just too constricted for a big implant to work or look natural - so some leeway/discretion for your surgeon is generally accepted and good. i would normally, however, agree a range based on your wishes and the practical limits of what your chest will accept and if there is any droop of the breast (if a lift not being done at the same time) do you know if your surgeon, for example, uses on table sizers? did you try a rice test to see what volume was your preferred?

It is not uncommon for the surgeon to make an implant selection during surgery because at that time all information is known to the surgeon about what will best fit the space. However, it is unusual that you were not given a range of agreed upon sizes that will be used. I encourage you to return to your surgeon to discuss your concerns. It is important that you feel comfortable and confident as you head into surgery. You also want to be sure that high profile is what is best for you. You should not be limited by your surgeon's preference for a single profile.
