Since you're only three weeks into your recovery, you can expect more reduction in swelling over the next several weeks. Wearing a compression garment is essential to minimize swelling and support proper healing. It’s too early to determine final results, as the firmness and swelling you're feeling are part of the natural recovery process.

As for the changes you're noticing with your erections, this is likely due to the swelling and tissue firmness compressing or affecting the surrounding structures. Once the swelling decreases, your erection length and ability to maintain erections should return to normal. However, if these symptoms persist beyond the healing period, I recommend following up with your surgeon.

Male patients can have penile swelling for several weeks after a lower body lift.  Typically, 50% of the swelling dissipates after 3 weeks, 80% of the swelling dissipates over 6 weeks, and the remainder of the swelling dissipates over the next 3-6 months.  Final results may not be appreciated until a year after surgery. Scrotal support with elevation and supportive garments can significantly improve the swelling.  Heat and massage can help with unwanted bruising in the area and help with secondary swelling.  Erectile dysfunction should improve as the swelling dissipates.
