I'm seriously looking into overhauling my body, at 19 I weighed over 305 pounds. Currently I am 57, 6', 200. I am confused as to what procedure(s) to ask for when requesting a consultation. Gynecomastia for sure, but would I go for liposuction, tummy tuck, lower body lift or panniculectomy? Thank you in advance or your guidance.
Answer: All of the above In order to correct current skin redundancy and gynecomastia, you will need several procedures that will include breast reduction for gynecomastia, abdominoplasty and liposuction of abdomen and flanks. In addition to gynecomastia reduction, you can also consider a tummy tuck with liposuction to the abdomen/waistline. Depending on your health status, your plastic surgeon may be able to do everything all at once. Good luck!

Your best answer will come from an in-person or virtual consultation with a board certified plastic surgeon who has a wealth of experience in performing procedures that can result in a leaner, firmer, and more youthful abdominal contour. Lliposuction/liposculpture of the abdomen, sides, and back could be performed to determine if a follow-on TT is required to remove lax skin. Your photos do not indicate a need for a panniculectomy. Though the conventional teaching has been that liposuction cannot remove breast tissue, I have found that I can remove the breast tissue with liposuction alone. This allows for small segmental reduction of the breast to produce the best contour possible and avoid over resection, cratering, and contour deformities. This also provides for much less scarring and potential poor scarring concerns.

Answer: Gynecomastia Surgery and... Thank you for your photos. You would certainly benefit from liposuction and glandular excision of the chest -- possibly with skin excision. It is difficult to tell how much fat you have around the abdomen that is amenable to liposuction. If the fat is stored above the muscles this can easily be removed with liposuction. However, if the fat is below the muscles (intra-abdominal fat) then liposuction will have minimal benefit. The only way to tell is through physical exam. Be sure to see someone who has experience in male plastic surgery and has a good reputation so you get the proper procedure.
