I had a rhinoplasty a bit over 2 years ago which was meant to straighten my nose which was left very crooked after an accideent. The surgeon thought he knew better and without my permission, he decided to give me the nose job he thought I needed. There's a lot about my new nose which I don't like, but probably the thing which bothers me the most is that smiling doesn't feel like it used to. I know I used to have a plunging tip, which made it feel like a smile spread out over my whole face, while now it feels like there's this pin at the base of my nose which get's in the way and makes it feel like I'm grimacing rather than smiling. So my question is, is it possible to somehow make my nose move the way it used to? Does this mean the strut would have to be removed, or is it somehow possible to have a plunging tip with a columellar strut? The surgeon did cut away a huge amount of cartilage from the alar cartilages so I worry there's not a lot of cartilage left to support the tip without this strut.

I'm sorry for what you have had to go through, now looking at your third rhinoplasty. I know it takes alot to make the decision to have a rhinoplasty in the first place, and for it to not go the way you expected can be extremely frustrating.

Hello and thank you for your question. Based on your photographs, you may benefit from a revision rhinoplasty. Maneuvers for your surgery would include adding dorsal augmentation with diced cartilage wrapped in fascia as well as tip derotation, among other maneuvers. With good surgical...
