It’s been a year since I had my surgery ( full anchor with 195cc? silicone implant) the scars do not bother me where the anchor is but my nipples look completely deformed.. they were never this big or uneven, I know it’s my skin that inevitably makes this happen and it’s not the doctors fault but I don’t know what to do, I still don’t want to be naked with the lights on

It is these ducts that are creating the inversion. It is likely that if you do anything that doesn't actually cut the ducts, you would have a high rate of failure. By far the easiest approach is to consider this surgery only after you are done with childbirth or you do not intend to breast-feed.
