Hi, I had a breast lift 2 years ago using the round block technique which left me with an enlarged areola and then I had an anchor revision, the shape is much better now but I'm left with high riding areolas, in your opinion would a breast augmentation help to make the areolas seem more centered or will it risk worsening the situation?

Answer: Correcting Breast Deformity After Breast Lift Surgeries Correcting high-riding areolae is very difficult. Also, following the round block and even the anchor lifting techniques, there may be too much weight of breast tissue in the lower center of your breasts, below the areolae. There isn't a reliable, long-term method to suspend that tissue upward behind the areolae. Therefore, I would suggest surgery to include removal of a wedge of your own breast tissue just below the areolae in the midline of both breasts (as this is the weight that sags first and most), tighten and lift the remaining tissue, and then place an implant centered behind each areola. This is called a Plus/Minus breast lift as the surgeon is removing some of your own tissue, but replacing it with an implant in a better position.

Answer: Mini lift with small implants Breast implants alone will not solve your problem. I recommend a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, small implants can be placed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.
